Sending patients Home: You Are Not Alone

An inconsistent discharge process in the hospital can leave patients with all sorts of uncertainties as they transition home. But one of the most powerful communications to patients can simply be letting them know “you are not alone.”¬†

Try this with the next patient you discharge to assure them that they are supported: Be sure to leave them with a phone or pager number they can use to reach someone at the hospital. Let them know, “We’re here for you after you go home should anything come up.”
